The GP 69 Form is a standard medical examination form used in the Republic of Kenya for candidates seeking employment, contract extensions, or registration with the Teachers Service Commission (TSC).  Purpose and Usage 

: Before seeing a doctor, Part 1 often requires a stamp from your Sub-County TSC office to verify your application status. The Medical Exam : You take the form to a recognized government doctor gp69 form

• Export customs declaration (if you used a freight forwarder).
• Overseas registration document showing the vehicle is now plated abroad.
• A statutory declaration made at a solicitor’s office, sworn under oath that you exported the vehicle on a specific date.

• V5C (Section 11 – 'Notification of Permanent Export') : This is the action form. When you sell a vehicle to someone outside the UK, you fill out section 11 of the logbook and send it to the DVLA.
• GP69 Form: This is the confirmation document the DVLA sends back to you. It acknowledges that they have processed your export notification. The letter specifically states: "Thank you for telling us your vehicle has been permanently exported" and includes the vehicle’s details and the date of export.
